This platform facilitates a health related e-service where both users/customers and the   Health Service provider; ABC Hospital can directly connect to purchase goods and services.

  • Customers: individuals who purchase or receive the end goods and services, including those who receive goods and services requested by another individual.
  • Health Service Provider: ABC Hospital and shall be responsible for all the services, facilities, accuracy of content provided herein.         
  • Users: Shall be any individual whether customer/End User or Health Service provider that uses this platform

*Minors shall not be permitted to use the platform, its apps or websites facilitated herein.

1. Scope        
This Privacy Statement shall apply to all individuals who use the Avenirhealth App or website or any other connecting tool that may provide for the purpose of facilitating health services between Customer and Health Service Provider.  

2. Collection of Information

This Platform shall collect information all users provide directly, such as when Users create or modify their account, request on-demand services, contact customer support, or otherwise communicate with the Health Service Provider. This information may include name, email, phone number, postal address, profile picture (if provided), payment method, orders or services requested (for delivery services), delivery notes, communication with the Health Service Provider or third parties and any other information the User provides this Platform.  

  • Data provided by Users include:

Customer/ Service Providers profile

This Platform collects data when customers/Users/Health Service Providers create or update their account on the Avenirhealth App/ Website. This may include their name, date of birth, email, phone number, login name and password, address, profile picture, payment or banking information details and verifications, NIC information and other government identification documents.

This also may include gender and/or occupation.

We may use the photos submitted by Users to verify their identities, such as through facial recognition technologies.

Information on Entities such as their employees’ details, Business registration number, qualifications shall also be collected to ensure that information provided to Users are accurate and reliable.

Background check and identity verification:

This Platform collects background and identity verification information especially of Users and any third parties to ensure the Application is at all times safeguarded.

Demographic data:

This Platform may collect demographic data about Users, including through User surveys and questionnaires  in order to customise the services and products or to upgrade the experience of the Platform.

User content:

This Platform collects all information submitted when Users contact customer support, provide ratings, complaints or compliments. This may include photographs, feedback, evidence, or other recordings provided by Users or third parties.

  • Data created during the use of our services:

Location data

This Platform shall collect location data when the Avenirhealth app/website is running in the foreground (app open and on-screen) or background (app open but not on-screen) of their mobile device.

The Platform shall use this data to enhance a User’s experience of the app or website, as it is essential to ensure that proper mapping is created to guarantee that services provided to the customer by the Health Service Provider is provided in a timely manner. Further, location data shall improve safety features, and prevent and detect fraud.

Transaction information

Transaction information related to the use of our platform and services provided by the Health Service Provider, including the type of services requested or provided, order details, delivery information, date and time the service was provided, amount charged, discounts, distance travelled, and payment methods.

Usage and Preference Information:

The Platform collects information about how Users and site visitors interact with the platform, preferences expressed, and settings chosen. In some cases, it shall use cookies, pixel tags, and similar technologies that create and maintain unique identifiers.

The Platform also collects data on access dates and times, app features or pages viewed, app crashes and other system activity, type of browser, and third-party sites or services used before interacting with our services, if permitted by the User and the law.

 

Device Information

The Platform may collect information about the Users mobile device, including, for example, the hardware model, operating system and version, software and file names and versions, preferred language, unique device identifier, advertising identifiers, serial number, device motion information, and mobile network information.


Communication Data

The Platform shall enable Users to communicate with each other via the platform provided by the mobile apps and websites. For example, Users can call, text, or send other files to each other and calendar notices. To provide this service, the Platform receives some data regarding the calls, texts, or other communications, including the date and time of the communications and the content of the communications. The Health Service Provider may also use this data for customer support services (including to resolve disputes between Users), for safety and security purposes, to improve products and services, and for research and analytics.

Log Information

When Users interact with the Services via the platform it collects server logs, which may include information like device IP address, access dates and times, app features or pages viewed, app crashes and other system activity, type of browser, and the third party site or service you were using before interacting with our Services.


** All information above shall be collected based on the settings and permissions of each user.

  • Data from other third-party sources:
  • Users or others providing information in connection with claims or disputes.
  • Publicly available sources
  • User feedback, such as ratings, feedback, or compliments.
  • Users or the general public participating in surveys, questionnaires, and promotions
  • Information on external payment providers or other social media platforms Users link with, to create accounts on the platform; App and website.
  • Partner transportation companies (for drivers or delivery persons who use the Platform via the Health Service Provider.  

3. How the Platform uses your information:

  • To perform services professionally. To collect, provide, personalise, maintain, and improve the platform by providing products and services that Users may require, and to ensure ease of accessibility to the platform by Users by using information such as the creation and updating of Users’ accounts, verification, facilitation of payment and services, etc.         
  • We use personal data to help maintain the safety, security, and integrity of the Health Service Provider and Customers, in order to ensure that our platform is a safe space for all Users and to further design an organisational structure that will meet the needs and demands of the Users.         
  • To screen Users before permitting their use of our platform services and at subsequent intervals, including through reviews of background checks, where permitted by law, to help prevent use of our services by unsafe, fraudulent Users.
  • To send the User customised service-related messages via email, phone or text. For example, the Health Service Provider may send you service-related emails relating to account verification, consultations, products and services and information that a User may have requested to be provided.
  • To additionally send Users information via email, post, phone or text about promotions or other offers we think may be of interest to you, including offers from third parties. Information about promotions and offers will only be sent to Users in accordance with their marketing preferences.
  • To solve Customer/Health Service Provider’s Complaints. The Platform uses the information that is collected (including recordings of customer support calls with notice to and the consent of the User) to provide customer support, including:        
  • Direct questions to the appropriate customer support person
  • Investigate and address User concerns
  • Monitor and improve customer support responses and processes
  • To create advertising content for the purpose of promoting the platform on third party websites or apps.
  • The Platform may use the data collected for testing, research, analysis, product development, pricing analysis and machine learning to improve the User experience. This helps the Platform to improve and enhance the safety and security of the platform, improve the ability to prevent the use of the platform for illegal or improper purposes, develop new features and products, and facilitate insurance and finance solutions in connection with the platform
  • The Platform may use the personal data collected to investigate or address claims or disputes relating to the use of the Platform or as otherwise allowed by applicable law, or as requested by regulators, government entities, and official inquiries.
  • To undertake troubleshooting, data analysis, testing, research, improving the service and developing new products and services.
  • To protect rights or property or the rights and property of the Health Service Provider and other third parties, and to detect and investigate illegal activities and breaches of any agreement we have with Users.        

4. Sharing of Information        
The Platform may share the information we collect about Users in order to ensure that the platform provides a customised service:        

  • Through our Services        

We may share your information:        

  • With Health Care Service Stakeholders ; to enable them to provide the product or Services you request/purchase. For example, the Platform may share details of the User such as Username, location details to provide the required service/delivery.        
  • With third parties; to provide the User the service requested through a partnership or promotional offering made by a third party or the Health Service Provider.
  • With the general public; if you submit content in a public forum, such as blog comments, rating of the platform, social media posts, or other features of the Platform that were viewable by the general public.        
  • With third parties with whom you choose to let the Platform share information, for example other apps or websites that integrate with the Platform, or those with an API or Service with which the Platform integrates.
  • With Health Service Provider’s subsidiaries and affiliated entities that provide services or conduct data processing on our behalf, or for data centralization and/or logistics purposes.        
  • With Consultants, marketing partners, Legal, Auditors and other third-party service providers who need access to such information to carry out work on the Platform’s behalf.
  • In response to a request for information by a competent authority if the Platform or the Health Service Provider is legally obligated to disclose in accordance with, or is otherwise required by, any applicable law, regulation, or legal process.        
  • With law enforcement officials, government authorities, or other third parties if the Platform reasonably believes your actions are inconsistent with the Platform’s  agreements, Terms of Service, or policies, or to protect the rights, property, or safety of the Platform, Health Service Provider, Users or other third parties;
  • In connection with, or during negotiations of, any merger, sale of company assets, consolidation or restructuring, financing, or acquisition of all or a portion of our business by or into another company.        

Further, for the purpose of Social Sharing Features        

  • The Services provided by the Platform may integrate with social sharing features and other related tools which let Users share actions they take on our Services with other apps, sites, or media, and vice versa.        
  • The feedback that Users leave on Apps/Website/Platform helps the Health Service Provider to improve its service, and to respond to customer needs appropriately and effectively. Thus, if a User posts a review of the platform through our Application or any other social platform, the Platform may post the review and may use the feedback for any marketing, advertising, analysis or promotional material.          
  • Analytics and Advertising Services Provided by Others        
    The Platform may allow others to provide audience measurement and analytics services, to serve advertisements on the Health Service Provider’s behalf across the Internet, and to track and report on the performance of those advertisements. These entities may use cookies, web beacons, and other technologies to identify a User’s device when they visit the Platform and interact with the services provided therein as well as when a User visits other online sites and services similar to the platform.

5. Your choices regarding your information:

  • The Platform shall retain User data for as long as necessary for the purposes described above. This means the Platform shall retain different categories of data for different periods of time depending on the category of User to whom the data relates, the type of data, and the purposes for which the Platform collected the data.         
  • The Customer can unsubscribe from promotional email communications  by clicking on the “unsubscribe link” provided in each email. Please note that you cannot unsubscribe or opt-out from service-related emails and SMS text messaging as these are critical to performing platform services (e.g., account verification, order confirmations and tracking information, changes/updates to features of the Service, technical and security notices, payment details).

Every User subscribed to the Health Service Provider via an App or website shall have a right to make a written request to the Health Service Provider or Platform to have his/her personal data erased, within twenty one working days from the date of such request, provided that the User understands that this would entail a complete unsubscription and cancellation of Account from the Platform.

However, kindly note that in some circumstances the Platform may have to retain certain information about the User as required by law, or for legitimate business purposes to the extent permitted by law. For instance, if a User has a standing credit or debt on their account, or if the Platform believes a User may have committed fraud or violated its Terms, the Platform or Health Service Provider may seek to resolve the issue before deleting the User’s information.

  • Location Information        
    The Platform shall request permission for the collection of precise location from a User’s device per the permission system used by their mobile operating system. If a User initially permits the collection of this information, they can later disable it by changing the location settings on their mobile device. However, this will limit the ability to use certain features within the platform.         
  • Contact Information         
    The Platform may also seek permission for the app’s collection and syncing of contact information from your device per the permission system used by your mobile operating system. If you initially permit the collection of this information,
    iOS Users can later disable it by changing the contacts settings on your mobile device. However, to our knowledge Android platform does not provide such a setting.        
  • Promotional Communications        
    Users may opt out of receiving promotional messages from the Platform by following the instructions in those messages. If a User opts out, the Platform may still send non-promotional communications, such as account verification details, Services requested or paid for, or any ongoing business relations.

6. Third party sites        
The Platform may, from time to time, contain links to and from the websites or applications of our partner networks, advertisers, affiliates or third parties. If a User follows a link/advertisement to any of these websites or applications, please note that these third parties have their own privacy policies and the Health Service Provider nor the Platform shall accept any responsibility or liability for these policies. It is therefore advised that Users check these policies before submitting any personal data.

7. Cookie Policy        
How we use Cookies:         
A cookie is a small file which asks permission to be placed on a User’s computer's hard drive. Once a User agrees, the file is added, and the cookie helps analyse web traffic or knows when they visit a particular site. Cookies allow web applications to respond to a User as an individual. The web application can tailor its operations to a User’s needs, likes and dislikes by gathering and remembering information about their preferences.

Thus, the Platform provides a User with essential features and services and for additional purposes, such as:

  • Use of traffic log cookies to identify which pages are being used. This helps us analyse data about webpage traffic and improve the website in order to tailor it to customer needs. This information is only used for statistical analysis purposes and then the data is removed from the system.
  • Recognizing a User when they sign-in to use our services. This allows the Platform to provide the User with product/service/content recommendations, display personalised content, recognize the User as a Prime member, enable the User to ease of purchasing, and provide other customised features and services.
  • Keeping track of your specified preferences of goods and services. This allows the Platform to honour the User’s customised preferences, such as whether or not a User would like to see interest-based ads. A User may set their preferences through their account.
  • Conducting research and diagnostics to improve the content, products, and services.
  • Preventing fraudulent activity.
  • Improving security.
  • Delivering content, including ads, relevant to the User’s interests on the Platform  and third-party sites
  • Reporting. This allows the Platform to measure and analyse the performance of services.

Kindly note approved third parties may also set cookies when you interact with the Platform/Health Service Provider. Third parties include search engines, providers of measurement and analytics services, social media networks, and advertising companies. Third parties use cookies in the process of delivering content, including ads relevant to your interests, to measure the effectiveness of their ads, and to perform services on behalf of the Health Service Provider.

A cookie in no way gives the Platform access to a User’s computer or any information about the User, other than the data a User chooses to share. A User can manage browser cookies through their browser settings. If a User blocks all cookies on their browser, neither the Health Service Provider nor third parties will transfer cookies to that particular browser. If a User does block however, the User may have to manually adjust some preferences every time they visit a site as some features and services may not work.

These are used to understand, improve, and research products and Services, including when a User accesses our website and related websites and apps from a computer or mobile device. For example, the Platform may use cookies to understand how you as a User are expending site features, and segmenting audiences for feature testing. The Platform and our partners may use these technologies and the information received to improve and understand how Users use websites, apps, products, services and ads.

Advertising

These cookies and pixels are used to deliver relevant ads, track email marketing or ad campaign performance and efficiency. For example, our ad partners may rely on information collected through these cookies to serve you ads that may be interesting to you on other websites. Similarly, our partners may use a cookie, attribution service or another similar technology to determine whether we’ve served an ad and how it performed or provide us with information about how you as a User interacted with them.


8. Updates        
The Platform/Health Service Provider may occasionally update this Privacy Policy. If  significant changes are made, Users shall be  notified in advance of the changes via the Platform, or through other means, such as email. We encourage Users to periodically review this notice for the latest information on our privacy practises.
