This platform facilitates a health related e-service where both users/customers and the Health Service provider; ABC Hospital can directly connect to purchase goods and services.

1. Scope
This Privacy Statement shall apply to all individuals who use the Avenirhealth App or website or any other connecting tool that may provide for the purpose of facilitating health services between Customer and Health Service Provider.

This Platform shall collect information all users provide directly, such as when Users create or modify their account, request on-demand services, contact customer support, or otherwise communicate with the Health Service Provider. This information may include name, email, phone number, postal address, profile picture (if provided), payment method, orders or services requested (for delivery services), delivery notes, communication with the Health Service Provider or third parties and any other information the User provides this Platform.

This Platform collects data when customers/Users/Health Service Providers create or update their account on the Avenirhealth App/ Website. This may include their name, date of birth, email, phone number, login name and password, address, profile picture, payment or banking information details and verifications, NIC information and other government identification documents. This also may include gender and/or occupation. We may use the photos submitted by Users to verify their identities, such as through facial recognition technologies. Information on Entities such as their employees’ details, Business registration number, qualifications shall also be collected to ensure that information provided to Users are accurate and reliable. |

This Platform collects background and identity verification information especially of Users and any third parties to ensure the Application is at all times safeguarded. |

Every User subscribed to the Health Service Provider via an App or website shall have a right to make a written request to the Health Service Provider or Platform to have his/her personal data erased, within twenty one working days from the date of such request, provided that the User understands that this would entail a complete unsubscription and cancellation of Account from the Platform.
However, kindly note that in some circumstances the Platform may have to retain certain information about the User as required by law, or for legitimate business purposes to the extent permitted by law. For instance, if a User has a standing credit or debt on their account, or if the Platform believes a User may have committed fraud or violated its Terms, the Platform or Health Service Provider may seek to resolve the issue before deleting the User’s information.

The Platform may, from time to time, contain links to and from the websites or applications of our partner networks, advertisers, affiliates or third parties.
